In the double blind study, researchers led by Dr Laura Koutsky of the University of Washington, Seattle, and colleagues randomly assigned 2392 women aged 16 to 23 to receive three doses of placebo or the HPV-16 vaccine. Genital samples to test for HPV-16 DNA were obtained at enrolment, one month after the third vaccination, and every six months thereafter. All of the women were free of HPV infection at the time of enrolment.

The women were followed for a median of 17.4 months after completing the vaccination regimen. Women who developed evidence of infection were referred for colposcopy, and biopsy tissue was evaluated for cervical intraepithelial neoplasia and analysed for HPV-16 DNA. The primary end point was persistent HPV-16 infection, defined as the detection of HPV-16 DNA in samples obtained at two or more visits.

The incidence of persistent HPV-16 infection was 3.8 per 100 woman years at risk in the placebo group and 0 per 100 woman years at risk in the vaccine group (100% efficacy; 95% confidence interval 90% to 100%; P<0.001). All nine cases of cervical intraepithelial neoplasia that was related to HPV-16 occurred among the placebo recipients. The researchers will continue to follow the women "for years."

Researchers have discovered dozens of types of HPV, but only two (type 16 and type 18) are implicated in cervical cancer.
