A friend of mine recently delivered her second child vaginally at an outer-suburban hospital and has had persistent lower back pain.  Her obstetrician administered her epidural.  I thought these were always done by anesthesiologists?  Are OB's qualified and permitted to do epidurals in the U.S.?

If the obstetrician is trained in it I think there should not be any problem. Anyway, in Malaysia, the anesthesiologist is the one who does it. I haven't heard of any obstetrician giving epidural anesthesia.
